Blood Pressure Exam

Responsibilities of this position:

  • Coordinate the total Plan of Care for the patient in the Home Care setting assessing, planning, implementing and evaluating.

  • Organize daily assignments, setting priorities based on individual and overall needs of the clients while traveling to each home as scheduled.

  • Provide leadership for the team by working cooperatively with other Darling's Home Care disciplines, physical therapy, occupational therapy, speech therapy and medical social services.

  • Educate the patient/caregiver in the management of their illness and promote the home health support systems to decrease re-hospitalization and utilization of hospital emergency services.

  • Balance your clinical talent with compassion and focus while meeting the needs of multiple patients throughout your day.

Sports Injury

Responsibilities of this position include:

  • Coordinate the total Plan of Care for the patient in the Home Care setting assessing, planning, implementing and evaluating.

  • Organizational skills, setting priorities based on individual and overall needs of the clients, collaborating with patients to create short and long-term goals

  • Provide leadership for the team by working cooperatively with other Darling's Home Care disciplines.

  • Educate the patient/caregiver in the management of their illness and promote the home health support systems to decrease re-hospitalization and utilization of hospital emergency services.

  • Balance your clinical talent with compassion and focus while meeting the needs of the patients.

  • Manages patients to improve their ability to move, reduce or manage pain, restore function, and prevent disability through means of direct or indirect treatments or referral to another provider as appropriate

  • Supervises Physical Therapy Assistants as assigned

Occupational Therapy

Responsibilities of this position:

  • Coordinate the total Plan of Care for the patient in the Home Care setting assessing, planning, implementing and evaluating.

  • Organize daily assignments, setting priorities based on individual and overall needs of the clients while traveling to each home as scheduled.

  • Provide leadership for the team by working cooperatively with other Darling's Home Care disciplines, skilled nursing, physical therapy, speech therapy and medical social services.

  • Educate the patient/caregiver in the management of their illness and promote the home health support systems to decrease re-hospitalization and utilization of hospital emergency services.

  • Balance your clinical talent with compassion and focus while meeting the needs of multiple patients throughout your day.
